The disappointing loaf is a specific object. Pale, heavy for its size, with a tight crumb and a faint sourness, and a bottom crust that has separated slightly from the rest. It is not a mystery and it is rarely the recipe. Four variables decide almost everything about a plain yeasted bread, and a bad loaf usually has one of them badly wrong.
Those four are temperature, water, gluten, and time, and they trade against each other. Warmer dough needs less time. Wetter dough needs less kneading and more folding. That is the whole system, and once you can see which one your kitchen keeps getting wrong, the fix is usually one change rather than a new recipe.
The dough was cold
Yeast is a temperature-driven organism. A dough finishing the mix somewhere in the mid-seventies Fahrenheit will behave roughly the way a recipe expects. The same dough at 65°F, which is an ordinary winter kitchen, ferments so much more slowly that a two-hour instruction becomes four, and most people give up and bake at two.
Control it at the mixing stage, when it is easy. Adjust the water temperature rather than the room: cool water in August, comfortably warm water in January. Hot water is the one real hazard, since water hot enough to be uncomfortable on the inside of your wrist is well on the way to killing yeast outright. If your kitchen runs cold, an oven with only the light on is a reliable proofing box.
The dough was drier than it looked
Flour is not a constant. Whole wheat drinks far more water than white because the bran competes for it. Flour that has spent a humid summer in a paper bag already carries some. And cup measurements vary by how the scoop was filled, which is why the same recipe produces a slack dough for one person and a stiff one for another. A scale removes this problem permanently and costs less than two bags of good flour.
Stiff dough resists rising because the gluten net has to stretch against friction. Wetter dough moves more freely, traps gas more easily, and bakes into a more open crumb. It is also stickier and less pleasant to handle, which is the honest trade. If your loaves are dense and your dough feels like modeling clay, add water in small increments across a few bakes rather than all at once.
The gluten never developed
Gluten is the elastic web that holds the gas the yeast produces. Without it, carbon dioxide escapes and the loaf stays flat no matter how alive the yeast was. It forms two ways: by working the dough, and by simply letting flour and water sit together, which does a surprising amount of the job on its own.
The low-effort route is a rest of twenty to forty minutes after mixing flour and water, then three or four sets of stretching and folding the dough over itself, spaced half an hour apart. That builds as much strength as sustained kneading and asks for about ninety seconds of attention each time. Test it by stretching a piece thin: if it goes translucent before it tears, the structure is there.
Flour protein matters here too. Bread flour carries more of it than all-purpose and produces a chewier, taller loaf. All-purpose works fine for most home breads. Whole wheat, no matter how well handled, gives a denser result because the bran physically cuts through the strands you are trying to build, so a mostly-whole-wheat loaf will always be tighter than a white one and that is not a mistake.
The rise was judged by a clock
Recipes give times because they have to say something, but dough responds to its own conditions. Judge the second rise by poking a floured finger about half an inch into the dough. If the dent springs straight back, it needs longer. If it fills in slowly and only part of the way, it is ready. If it stays as a hole and the surface sighs, it has gone too far.
Overproofing is the failure people misdiagnose most often, because the dough looked magnificent an hour ago and then collapsed in the oven. The gluten has been stretched past what it can hold and there is no un-doing it. A loaf caught at that point still bakes into something edible; it just comes out flat and slightly boozy.

The bake is where crust happens
A lean loaf wants a genuinely hot oven, and it wants steam for the first stretch of the bake. Steam keeps the surface flexible while the loaf makes its final expansion, and only then does it set into a crust. Home ovens cannot inject steam, so the practical answer is to bake inside a covered pot for the first twenty minutes or so and uncover it to color. The dough makes its own steam and the lid keeps it there.
Score the top with a blade held at a shallow angle just before it goes in. That gives the expansion somewhere deliberate to go, instead of tearing open at whichever seam happens to be weakest. And bake it further than instinct suggests. Lean loaves are done when the interior reads somewhere around 205°F, and a loaf that looks done at fifteen minutes of color usually is not.
Cooling is part of baking
The crumb of a loaf is still setting when it comes out of the oven, and cutting into it early gives you a gummy, compressed center that no amount of technique elsewhere will fix. Wait until it is barely warm at least, and for a big loaf, wait until it is properly cool. The smell coming off the rack while you wait is real and unfair, and the bread is measurably better for it.
Once cool, keep it cut side down on a board or in a paper bag for a day or two. The refrigerator is the worst option, since bread goes stale fastest at refrigerator temperatures. Anything you will not eat within two days should be sliced and frozen, and a stale loaf can be brought most of the way back by wetting the crust briefly and putting it in a hot oven for ten minutes.
If you change one thing this week, change the water temperature and buy a scale. Those two account for more sunken, heavy loaves than every technique argument on the internet put together.
